Why this decision matters
Guessing a cause or repeatedly clearing the indication can remove information useful for diagnosis.
Work through the actual task
- Record the door, time, role and reader indication.
- Report whether access was denied or granted without a physical release.
- Provide the model and a safe photo of the fault display. Keep passwords and access codes out of the message.
Include these facts in the report
- Exact equipment model and the real location affected.
- The visible message, when it started and whether the fault is intermittent.
- Functions that still work and any recent power, network, account or building change.
- The person authorised to arrange access and approve diagnosis.
Worked example
One credential fails at one door but works elsewhere. This is a hypothetical example, not a completed customer installation.
The useful evidence at the end of this example is: A fault statement separating permission, controller and door-mechanism symptoms.
Limits and follow-up
Door security must preserve safe exit. Have the appropriate qualified professionals confirm the door and emergency release design.
If the relevant check cannot be completed, record what remains uncertain and who will resolve it before work or sign-off proceeds.
